3. Acute Medical Emergencies Protocol (Immediate 911 Directive)
If you are experiencing acute medical symptoms—including but not limited to sudden crushing chest pain, shortness of breath, unexplained numbness or weakness, sudden speech difficulty, severe trauma, loss of consciousness, or sudden loss of bowel or bladder control—call 911 immediately or proceed to the nearest hospital emergency room. Never delay seeking emergency medical treatment because of information read on this website.
4. Outpatient Appointments, Scheduling & 24-Hour Cancellation Policy
Mission Health & Wellness operates on an unhurried, one-on-one care model. When you schedule an appointment with Dr. Fred Lewin or Karise Lewin, LMT, dedicated treatment time is reserved exclusively for you.
- Advance Notice Requirement: We require a minimum of 24 hours advance notice for any appointment cancellation or rescheduling. This allows us to offer the reserved clinical time to another patient in pain.
- Late Cancellations & Missed Visits: Missed appointments or cancellations provided with less than 24 hours notice may be subject to a standard missed appointment fee.
- Arrival & Punctuality: Please arrive 5 to 10 minutes prior to your scheduled session to ensure you receive your complete allocated treatment time.
5. Fees, Payment Policies & Insurance Superbills
Mission Health & Wellness is committed to complete financial transparency. We operate as an independent, out-of-network healthcare clinic to maintain the freedom to provide unhurried, personalized restorative care without insurance-imposed treatment restrictions or arbitrary visit caps.
- Payment Due at Service: Payment for all clinical services, manual therapies, and cold laser sessions is due in full at the time services are rendered. We accept all major credit cards, debit cards, cash, and Health Savings Account (HSA) / Flexible Spending Account (FSA) cards.
- Insurance Superbills: While we do not bill insurance companies directly, we gladly provide detailed, itemized receipts (Superbills) containing standard CPT procedure codes and ICD-10 diagnostic codes upon request. Patients may submit these Superbills directly to their private insurance carriers for potential out-of-network reimbursement.
6. Intellectual Property, Trademarks & Permitted Educational Use
